In 2022, the Swedish Women’s Hockey League (SDHL) made a historic shift, becoming the first top-tier women's league globally to permit body-checking. While the change was intended to align the sport with international play, it has ignited a significant debate regarding player safety. Recent research from Lund University has provided the first empirical data on the consequences of this policy, tracking injury rates over a six-season period to determine if the sport’s increased physical intensity has come at a cost to its participants.
The data, published in Sports Medicine - Open and the European Journal of Sport Science, highlights a stark reality: injury frequency has surged since the rule change. In the season prior to the policy update, the league recorded 6.6 injuries per 1,000 player-hours. Following the introduction of body-checking, that figure spiked to 16.7 per 1,000 player-hours. While the numbers have shown some stabilization in the years since, the average remains significantly higher than in the pre-check era, serving as a critical indicator for leagues in the U.S. and Canada currently monitoring Sweden’s trajectory.
The Nature of Modern Hockey Injuries
The transition to a more physical game has changed the anatomy of match-related injuries. Researchers noted a clear correlation between the introduction of body-checking and the prevalence of specific traumatic incidents. Shoulder and knee injuries are now among the most frequent insurance claims, likely resulting from the mechanics of checking into the boards. Furthermore, the league’s medical staff reported that body-checking has become the leading cause of injury, with concussions emerging as a significant, recurring concern for players.
Perhaps most alarming is the behavioral data surrounding these injuries. The study revealed a culture where athletes often return to the ice despite having suffered a concussion—a finding that underscores the need for better on-ice medical oversight. A high concentration of injuries occurs during the third period, pointing to a potential mix of physical exhaustion and the unique pressures faced by elite female athletes, many of whom balance professional training with external employment, which may hinder adequate recovery.
