Coronary artery calcium (CAC) scans have become a staple in modern preventive cardiology. By using a CT scan to detect calcified plaque within the arteries that supply the heart, physicians aim to gauge a patient's risk of future cardiovascular events. However, a significant new study published in JAMA and led by researchers at Northwestern Medicine suggests that these scans are not a universal solution. After tracking over 6,000 adults for a decade, the study indicates that for many people, these tests may offer limited additional value beyond standard clinical risk assessments.
The study specifically compared the predictive utility of adding a calcium score to the American Heart Association’s PREVENT cardiovascular risk calculator. While the calculator uses established markers like age, sex, blood pressure, and cholesterol, the calcium scan adds a physical look at the arteries themselves. For the broader population, the researchers found that the addition of the calcium scan only marginally improved the ability to predict heart attacks and strokes.
Targeting the Intermediate Risk Group
The research highlighted a critical shift in how doctors should view these scans: they are most effective as a tie-breaker for patients caught in the clinical "gray zone." For individuals already categorized as high risk, the scan often provides little new information, as these patients typically require preventative measures like statins regardless of their calcium score. Conversely, for those at very low risk, the scan can lead to unnecessary radiation exposure, anxiety, and added healthcare costs without a clear clinical benefit.
The study found that the true value of the calcium scan emerges for patients who fall into the borderline or intermediate risk categories—those estimated to have a 3% to 9% risk of heart disease within 10 years. In these cases, the scan serves as a vital diagnostic tool to confirm whether a patient’s actual risk is significantly higher or lower than initial calculations suggested. This precision allows clinicians to make more informed decisions about whether to initiate long-term preventative treatments.
