Presented at the European Respiratory Society (ERS) Congress in Barcelona, three major studies conducted in Austria and Ireland highlight a shift in how nicotine is being consumed. Researchers argue that the decline in cigarette use creates a false sense of security, masking an underlying surge in overall nicotine dependency that is now reaching younger demographics than ever before.

The Austrian Longitudinal Study

A comprehensive study led by Dr. Charmaine Helk-Lim from the Ludwig Boltzmann Institute for Lung Health analyzed data from over 15,000 Austrians between 2012 and 2024. The data paints a stark picture: while traditional smoking rates dropped from 24.2% to 16.3%, the prevalence of "alternative" nicotine products, such as vapes, skyrocketed from 0% to 17.2%.

Perhaps more alarming is the rise in "dual use," where individuals utilize both traditional cigarettes and electronic devices, which grew from 5.2% to 18.3%. The study also identified a significant drop in the age of initiation. Individuals born in the 1930s typically began smoking around age 20, whereas those born in the 2000s are starting nicotine use by age 15. Furthermore, smoking rates among those under 18 have nearly doubled, jumping from 2.6% to 5.0% over the study period.

The Irish Youth Trends

A second research team, led by Professor Joan Hanafin of the TobaccoFree Research Institute Ireland, examined the behavior of over 5,000 Irish adolescents aged 15 to 16. Their findings suggest that the trend is not an isolated incident but a broader European pattern. Between 2015 and 2024, smoking rates among this group saw a modest decline, but vaping rates followed a more volatile trajectory, peaking at 18.1% before settling at 15.7%.

Crucially, the study found that 76.2% of vapers in 2024 had never smoked a traditional cigarette before, suggesting that vaping is increasingly serving as a gateway rather than a cessation tool. A longitudinal follow-up by Professor Luke Clancy on 3,380 young people tracked them from age 13 to 25. This study confirmed that nicotine habits tend to intensify into early adulthood, with 38% of 25-year-olds reporting current e-cigarette use. The data indicates that the risk of becoming a smoker by age 25 is significantly lower for those who never experimented with vaping in their youth.

The implications for public health are profound. Experts point out that nicotine remains a highly addictive substance, and the long-term impacts of e-cigarette vapor on lung and cardiovascular health are already beginning to surface in clinical literature. The "success" of reducing cigarette numbers is being neutralized by the ease of access to high-nicotine vaping products.
