In a significant shift for lung cancer therapeutics, researchers have announced transformative results from the DESTINY-Lung04 phase 3 trial. The study, presented at the 2026 World Conference on Lung Cancer, reveals that the antibody-drug conjugate trastuzumab deruxtecan (T-DXd) provides a superior clinical outcome as a first-line treatment for patients suffering from advanced or metastatic HER2-mutant non-small cell lung cancer (NSCLC). Historically, this patient population has faced a poor prognosis, often showing limited response to traditional immunotherapy and chemotherapy combinations.
The trial represents the first global phase 3 study to confirm that a single-agent HER2-directed therapy can outperform the standard-of-care regimen of pembrolizumab combined with platinum-based chemotherapy. By directly targeting HER2 mutations, T-DXd offers a more precise intervention, addressing a long-standing gap in pulmonary oncology care.
Understanding the Clinical Outcomes
The trial's primary endpoint, measured by blinded independent central review, showed a median progression-free survival (PFS) of 14.3 months for patients treated with T-DXd. In contrast, those receiving the standard immunotherapy and chemotherapy cocktail achieved a median PFS of only 8.3 months. This six-month delta indicates a statistically significant improvement, offering patients a longer window of stability and disease management.
Beyond progression-free survival, T-DXd demonstrated notable strength in objective response rates. Patients in the T-DXd cohort recorded an objective response rate of 70.0%, compared to 44.5% in the control group. Furthermore, the duration of response was significantly longer for the T-DXd group, standing at 13.4 months versus 9.7 months for the standard-of-care cohort. These metrics collectively reinforce the potential of T-DXd to become a foundational treatment for treatment-naive patients.

While this is higher than the incidence rate observed in the control group—which stood at 2.3%—researchers suggest that the drug's efficacy benefits outweigh the management of these side effects when monitored appropriately.
Overall, the rate of grade 3 or higher drug-related adverse events was largely comparable between both study arms, with 34.1% in the T-DXd group and 33.6% in the pembrolizumab/chemotherapy group. These findings indicate that while T-DXd introduces specific pulmonary risks, it maintains a manageable toxicity profile, positioning it as a viable and effective first-line standard for those diagnosed with HER2-mutant lung cancer.
