In a significant advancement for interventional cardiology, new findings from the ISOLEDS trial have provided definitive evidence that optical coherence tomography (OCT) is a viable and noninferior alternative to intravascular ultrasound (IVUS) when guiding complex stenting procedures. The study, presented at the recent ESC Congress 2026, focused specifically on patients suffering from distal left main bifurcation lesions—a condition where the primary artery supplying the heart's left ventricle is narrowed at a critical branching point.
Percutaneous coronary intervention (PCI) requires immense precision, especially when navigating these bifurcations. Traditionally, clinicians have relied on IVUS to visualize the vessel and ensure optimal stent placement. However, OCT offers a distinct advantage with its superior resolution, providing a clearer look at the vessel lumen. By proving that OCT performs just as well as the industry-standard IVUS, medical professionals now have greater flexibility in choosing the best diagnostic tool based on specific patient anatomy and institutional expertise.
Understanding the Trial Methodology
The ISOLEDS trial was an extensive, multi-center, open-label study that spanned 24 clinical sites across China. Researchers enrolled 664 patients diagnosed with true distal left main bifurcation lesions, randomizing them into two cohorts: those receiving OCT-guided PCI and those receiving IVUS-guided PCI. The trial followed these patients for 12 months, monitoring for target lesion failure, which included cardiac death, related myocardial infarction, or the need for follow-up revascularization.
The results were compelling, with the 12-month Kaplan–Meier estimates for target lesion failure showing 14.4% for the OCT cohort compared to 19.6% for the IVUS cohort. These figures, supported by rigorous adjusted hazard ratio analyses, demonstrate that OCT is statistically noninferior to its counterpart. The consistency of these results across various sub-analyses bolsters the clinical confidence in adopting OCT for high-stakes cardiovascular interventions.
