In the high-stakes world of out-of-hospital cardiac arrest (OHCA), time is the ultimate arbiter of life. For years, medical systems have leveraged smartphone-based volunteer networks—often called community first responders—to bridge the crucial gap between the moment a person collapses and the arrival of professional emergency medical services (EMS). The premise is intuitive: alert nearby, trained, or willing volunteers to perform cardiopulmonary resuscitation (CPR) and deploy Automated External Defibrillators (AEDs) to save lives when seconds count.
However, new data from the HeartRunner trial, unveiled at the ESC Congress 2026, has provided a rigorous, reality-check evaluation of these systems. As the first large-scale randomized trial to test this intervention, it sought to determine if the deployment of community responders actually improves 30-day survival rates. The findings indicate that while these programs are undeniably effective at increasing life-saving interventions, their impact on long-term survival metrics is more nuanced than previously assumed.
The HeartRunner Trial Methodology
Conducted in the Capital Region of Denmark, the HeartRunner trial analyzed 2,060 suspected cases of out-of-hospital cardiac arrest. The study utilized a randomized 1:1 design, comparing instances where community responders were alerted alongside standard EMS versus cases where only the standard EMS response was deployed. Researchers stratified these incidents by the estimated travel time of the volunteers, creating two groups: those within a three-minute radius and those within a three-to-nine-minute window.
The study sought to achieve two primary goals: measure the impact on patient outcomes and verify the safety of utilizing untrained volunteers. By standardizing the response across a large cohort in an urban setting, the researchers were able to filter out anecdotal success stories and look at the statistical cold reality of survival outcomes.
Key Findings: Efficacy vs. Outcomes
The results present a complex picture. While the activation of community responders successfully increased the rate of bystander CPR and doubled the rate of bystander defibrillation—significant operational wins—it did not statistically shift 30-day survival rates. Specifically, in cases where responders arrived within three minutes, survival remained at 15% with community intervention versus 17% without. Similar neutral findings were observed in the three-to-nine-minute arrival stratum, where survival was 13% with responders versus 14% without.
